Add support for Lens names #30

This commit adds support to show the Lens name in the breadcrumbs.

It does NOT add support for resources under the lens, unfortunately, that is still under investigation in https://github.com/Formfeed-UK/nova-breadcrumbs/issues/16 it seems.

But for now at least the lens name shows up at the end of the breadcrumb list.

Thanks for this, ill write some tests for it on the test repo and get it merged asap
