No icon appears in the browser's toolbar. #213

What steps will reproduce the problem?
1. Install latest version (0.16) and restart the browser to load it.
2. Add a Nagios site (with nagiosadmin user).
3. Enable above site.

What is the expected output? What do you see instead?
I'd expect an icon in toolbar in order to watch/access the extension.
Instead of that, no icon appears.

What version of the product are you using (Nagios Checker and Nagios)? On what 
operating system?

Iceweasel (Firefox) 31.6.0 in Debian GNU/Linux 8.0 (jessie) 64-bit.
Nagios 3.5.1 and Apache 2.4.7-1ubuntu4.4.

Please provide any additional information below.

I've looked for the icon in all browser's settings without success. I've 
changed display settings in extension's settings in order to reload visual 
aspects and restarted the browser several times.

Extension was unable to detect my CGI URL at first time; I've set it up 
manually and after that it was found yet.

Just for the record, it seems that another add-on (The add-on bar [0]) is 
needed.
