EHR 0.9.9 Usermanager cannot edit user's phone numbers & email #56

UserCreatorWizard allow a user to register 3 phone numbers, 1 fax number and 1 email but they cannot be edited later on by usermanager.

Was issue n° 145 in FreeMedForms: https://github.com/FreeMedForms/freemedforms/issues/145
