Release GutHub workflow / action - add package name to release title and link to CHANGELOG #31

Notifications about releases, e.g. to Slack will look much better and clearer if release titles contained package name and version, instead of just "Release 3.2.1".

Also, add a changelog link to task's changelog to release description and a list of issues resolved.
