I checked some of the failed transactions and it seems the error is "out-of-gas" with 46k gas spent. This is weird because other approvals had passed with ~26k gas. I'm not sure if increasing the "gasLimit" for approvals, similar to how we do for other txns, will fix the issue, but we can try and see if we have it again.
