Rewrite the entire module to fit new standards

The entire module has to be rewritten and will meet the following new standards:

• Simple build system without gulp, similar to dnd5e's and scriptable-item's.
• Only plain JS
• LESS for stylesheets
• Handlebars for templates + ApplicationV2 usage
• Data models for data, with migration from old data
