Fixes:

The membership form of the "Join" page is no more collapsed by default
The contact form has been removed from the "Join" page to avoid confusing it with the membership form

@mariaveldhuizen I just moved some of the repetitive code to another "include" file so that both "default" templates are easier to maintain. If you see no modifications to add, tell me and I'll merge this branch! Good work 👍
