Duplicated code in `icu_entry` #144

Lines 171 - 186 in icu_entry.R are duplicated (see lines 189 - 203) and should be removed. This does not affect the output of the function, but leads to a (potentially confusing) duplicate warning message when as_outcome = TRUE.
