Create fancy issue template for MAPL #1775

The folks at ESMF (hat tip @rsdunlapiv and @billsacks) have a cool issue template which points to both "Open a new issue" as well as "email us for other things":

This seems like a good idea and we, ahem, "borrow" it for use here and in other repos (like GEOSgcm).
