Create basic UI to allow the user to specify the server url, username and password #10

Start with some basic text boxes. Also allow the users to specify the folder where the data is downloaded to.

Basic text boxes UI is now available. The next will be to use a similar UI as the other QGIS connector such as PostGIS where several Geoserver urls could be remembered
