Add reporting data from cities' teams #24

We discussed the idea of designing a web form where each team can periodically report on activities and information that cannot be extracted from the OSM database - for example, the number of people who attended mapathons.

Would be great to have the output from this form automatically feed statistics and reports on the Open Cities website. Using a simple Google Form may be an option to output to JSON for integration in city pages.

I'll let @ppasq research and propose some options from the technical side.

Mira will lead the design of the form based on M&E requirements and discussion with HOT

@gracedoherty @nualacowan let us know if there's an output from the form that we can review and if there's consistent data to use. We could then consider a dynamic visualization/report to add to the website.
