Name component

A form builder user wants to ask their user to provide a name.

Determine if we should revise component to use USWDS standard: https://designsystem.digital.gov/patterns/create-a-user-profile/name/

Generic fieldset should be able to converted into a name component.

• [ ] Figure out what a "pattern" is from the form builder user's perspective (not the architectural version of a pattern)
• [ ] Design UI affordance for user to implement common sets of inputs
• [ ] Figure out level of flexibility to offer form builder users in early version of the product
• [ ] Make sure team is clear on terminology
• [ ] Strong bias toward stealing things wholesale from USWDS
