Evaluation Form: Delete

User story

As a challenge manager, in order to maintain a manageable list of evaluation forms, I want to be able to remove the forms that are no longer needed.

Acceptance Criteria:

• [ ] A challenge manager can delete an evaluation form if it is not within or after evaluation period (until challenge end date)
• [ ] When the form is deleted it is disassociated from the challenges and challenge phases it was assigned to

@r-bartlett-gsa does this Story only refer to the Draft forms, or can a user delete Forms for previous Challenges that are completed? If it's only for Drafts, we don't really have drafts now so this card can be deleted?

@stepchud The user can delete the form until challenge end date. I updated the AC and removed any mentions of 'Draft'. This card is still needed and should not be deleted.

Upon discussing this with Stephen, we postponing Delete option, since it might not be needed. Challenge managers will only be able to create valid forms, and one form per challenge, which should not result in unnecessary forms.
