Create touchpoints form that includes source of feedback (catalog page)

User Story

In order to get organized feedback at a dataset level and to improve efficiency in handling user questions, Data.gov team wants to create a new Touchpoints form from catalog pages

Acceptance Criteria

[x] GIVEN I am on catalog page for a dataset AND there is button to provide feedback WHEN I click on that button I am presented with a Touchpoints form EITHER via a modal or on a new page THAT will include a location_code with the name of the dataset, which will be transmitted to Touchpoints once the form is submitted
[x] GIVEN I am a Touchpoints admin AND I view the results page for the feedback form THEN I will see submissions that include the location_code for the dataset
[x] GIVEN I am a Touchpoints admin AND I view the results page for the feedback form THEN those results will be sortable via that location_code
[x] GIVEN I am a Touchpoints admin AND I view the results page for the feedback form THEN I will be able to export those results as a CSV

[x] Update the dataset template to ensure the Contact button points to the new form URL
- [x] Ensure that the form opens either in a new tab OR in a modal
- [x] Ensure that the form URL includes the location_code as a query string parameter (ex. https://touchpoints.app.cloud.gov/touchpoints/1327b00b/submit?location_code=dataset-slug
- [x] Ensure that the dataset_slug in the above example is actually the CKAN human readable URL (ex: https://catalog.data.gov/dataset/electric-vehicle-population-data = electric-vehicle-population-data

trying to check out touchpoints. when i attempt to login i get an error. not sure if this will block this ticket but wanted to pass along the info.

Good point. We might need to create a user for whoever works on this.

@rshewitt I didn't find you as a user, and couldn't add you as a user. Do you see anything in your GSA email that may have been an invite from Touchpoints?

My account was deactivated. Sent an email to feedback-analytics@gsa.gov to get it reinstated.

@hkdctol I was invited to touchpoints last march ( checking gmail ). I figure I signed up during onboarding but can't recall. I'll reach out to feedback-analytics@gsa.gov

We should have this component enabled with USWDS: https://designsystem.digital.gov/components/modal/

We can use the modal generated by Touchpoint script to keep things simple. The new form is installed on catalog-dev.data.gov for preview.
