OASIS+ DPA form

As a user, I need to fill out the OASIS+ DPA form, in order to receive authority to execute a task order on of the OASIS+ contracts.

Acceptance criteria:

• Seamless experience for users and not bouncing them from traditional gsa.gov look and feel
• Different than existing OASIS DPA form, to prevent confusion
• Successfully sends data to Drupal backend
• Form should reflect the approved fields
• Lives as child page to existing DPA page with some clear way to link there

• New child page nested under Obtain a Delegation of Procurement Authority
• Almost all fields in their preview form are required. We removed the optional fields and just added a line "All fields are required." If those optional fields really are needed (middle name, bureau, fax number), we can add them back in, but as a form best practice we're trying to only ask for information that is really needed.
• Added section heading and reordered the fields
• Changed the radio sets to checkboxes for all of the acknowledgement items. Since these fields were required, the users should be required to check the box to acknowledge the item and not have Yes/No options.

@klohman I like the look of this page providing they don't need it to be different to get into the DPA tool. We'll hear more once @mark-meyer hears back. Thank you for getting this going quickly.

May 31st update: Mark and Jon met with OSC and GSAIT, who could not figure out why our form design wasn't working, as nothing was showing up in the error logs. They're going to build an OASIS+ DPA form on Drupal / GSA.gov. Moving this card to done.

We have figured out how to get data from a form on OASIS+ page to the GSA system. We need to write some validation code to validate user input before submitting the form. We will work on this when we are back from the off-site meeting.

@PowderD we're going to merge from staging to production today. Could you do a test when we're in production to make sure things flow into the DPA database, as expected?
