PIV App Integration Models #77

Submitting initial conversion of notes found in PIV issues entry #35 into markdown (.md) file. Did some cleanup and slight re-organization, Content still need lots of build out as embedded note/reminders indicate. I can assist subject matter expert in doing so if you point me to the right/best person. Hopefully this conversion helps continue momentum of creating this playbook.

IMO a PACS using PIV fits this, in that it's the "role mapped externally" use case using X.509. Are you looking for contributions here?

Can you move this markdown under the _devconfig? And do a pull request to the staging branch instead of gh-pages branch? We're working on testing the integrations with Federalist (for hosting) and gh-pages will be removed during the deployment
