As a user visiting any benefit finder form, if the form data is unavalible I would like some display of communication to let me know where else I can go to find information

Reason / Justification

We recently experienced an outage on production where requests to the s3 were 404' , any user visiting would have seen an empty page

Acceptance Criteria (AC)

[ ] request to form data 404' results in content communication to users visiting site,

option 1, low LOE:

"Application not available at this time. Please try again later or contact support for assistance."

option 2, high LOE:

"Application not available at this time. Please try again later or contact support for assistance."
