Missing URLs to investigate

This can be a running issue where we gather reports of Why isn't X url in the target URL list?. With each, we should:

• confirm that it is missing
• confirm that it actually should be in the list
• investigate why it's not in the list
• correct any issue that caused it not to be in the list
• update the list so that it is now present
