Updating README with dev and test setup details

This PR updates the README file for clarity about setting up for development and testing. Specifically:

Update instructions for .env file to include information about setting up development email (via ethereal)
Update .env instructions to specify locally hosted BASE_URL
Update database setup, migration, and training quiz import instructions
Update node enviroment setup instructions
Update testing instructions
