Create table or modify table to store the data from GSPC #553

We need the following information in a table to be able to be queried to generate the GSPC reports:

User's Full Name (this would only be applicable if they were issued a certificate)
User's Agency (this would only be applicable if they were issued a certificate)
User's Bureau (if applicable)
User's email address
User's selection of each of the questions
