• [ ] For the day-long intervals, e.g. gregorian-interval/2020-06-28T00:00:00/P1D, use the day/2020-06-28 form instead as it'll get a better label.
• [ ] Change the URI template to http://reference.data.gov.uk/id/{+period} -- remove the day/ part of the path as that'll be in the CSV values, and add the + character to avoid escaping / characters when substituting the value.
• [ ] Area codes need to be upper-cased.

For the cause-of-death codelist, what's the difference between "All causes" and "All deaths"?
