Form Redesign Frontend

Issue Number(s): #165

Implemented frontend redesigns for forms including required and optional questions, and grouping similar questions into the same boxes. Mobile views have been implemented for forms as well including the Confirm Cancel modal.

IMPORTANT: If there are any questions that should be grouped differently or should have a different required/optional status let me know.

Oooh it looks so good! I have a few minor nitpicks. The error messages between the radio buttons and input fields are inconsistent. For radio buttons the text and icon are black but for input fields they are red. Can we make all of the text black for consistency? And there seems to be a little too much space after the save button. Can we decrease the space just a little bit (maybe around half of the current space)? Can we also add a confirmation modal if you click "return to dog"? It can be the same modal that appears if you click the cancel button. I don't think that was in the Figma, so that's definitely on me. Other than that it looks awesome. Thanks for your hard work!!
