Add report error link on encounter pages #170

On encounter page, would be nice to have a "report an error" link on the page. Simply can link to contact page or ideally, link to encounter page with following data already included in the text body:

Username: [AUTO] Log File Name: [AUTO] EVTC Version: [AUTO] Uploaded: [AUTO] Encounter URL: [AUTO] Additional Comments:

I think under the current encounter summary info in the top right makes sense: image

Can you add some text to the following effect on the form page:

"Please include a link to your log file if possible"
