Basically if you pick the custom Item Set (it doesn't matter if you select any items or not), go to the stage/loadout confirmation screen then back out from it, you will be put back to the Custom Item Set menu, but the game actually thinks that you are in the normal Item Set menu (where you can select Easy/Normal/Hard/Custom item sets), which makes the navigation rather confusing and causes some visual oddities.
Although you can still select menu options normally and going back to character select screen fixes the issue, so it is more of a visual bug. Also backing out from stage/loadout confirmation screen while picking the Easy/Normal/Hard item set works fine.
