Replace all E40k difference notes with footnotes. #38

I experimented with a few approaches, and managed to find something that seems to work across various situations and within the constraints of Asiciidoc and Antora.

I think I could go ahead and publish this now — but it wouldn't hurt for you to cast your eyes over it, if you find the time. If you can't get to it anytime soon I'll probably go ahead, but let me know if you'd like me to hold off.

Once we've published this, I'll pull the tweaks into ongoing PR #36 so that's up to date and easier to merge to the master branch later.

I think these changes work well enough, so I'll go ahead and merge into master rather than have this hanging around and getting out of sync.
