Some headings should appear at the root level of PDF bookmarks #19

Some headings, such as "Notes", "References", and "Biographical Sketch" are indented in PDF bookmarks. They should appear at the root level of PDF bookmarks.

I also moved "Notes" to the root level of the table of contents. The ASU format manual does not show an example of "Notes" or "Biographical Sketch" in a table of contents, so expectations for these headings in the table of contents are unclear. However, one user reported that the Graduate College wanted "Biographical Sketch" to appear as a root-level heading in the table of contents. With this change, I am guessing that the same is true for "Notes."

In any case, the level of the headings in the PDF bookmarks has been changed to match with this update: 4e6be560631bc07f9cea253fe6df2486e782d400
