Extension crashes all the time

-when we click on save and continue button after filling the action form (if Abby did the right thing (the one you just demonstrated), will she know that she did the right thing and is making progress toward her goal?)

Blank screen no option to exit

updated the action.html file under template folder issue resolved and verified
