Add a changelog and links to previous versions #21

Start the changelog in doc/changes.rst and include a page linking to other versions of the documentation. The version number for the first release will be 0.1.0.

@seisman after #20 is merged, should we make a release? I want to use this in the GMT website a bit and it would be nice to have it officially release. This PR adds a changelog and links to other versions of the docs (latest, dev, etc). It also updates the install instructions for pip and conda.

I'm thinking the release should be 0.1.0 since we might make major changes to this in the near future.
