Use GitHub issue form with LinkML slot attribute names in PR #558 #678

There's a PR #558 from @jfy133 which seeks to use GitHub forms while making issue templates/creating issues. We are interested in adopting that pattern/technique.

We need to modify the language in his PR to use LinkML slot attributes. See here for agreed upon mapping.

I don't understand. We can hide implementation details from people who wish to submit terms. We expose MIxS metamodel terminology, e.g. extensions, checklists, structured comment names, etc
