Add missing checklist only data collection templates #734

PR #733 did not include data collection templates (Excel files) in mixs-templates/ for just the checklist itself even though all templates are organized by checklist.

For example, mixs-templates/MigsBa did not include the MigsBa.xlsx file.

This PR seeks to correct that.
