Update Health_PCPs.md #104

I changed some variable names to standardized ones. This work is a part of updating metadata files for the 2.0 release. I am still wondering if "state", "county", and "tract" should be updated or not because there are no appropriate names in the dictionaries.

Yeah, I will go back and handle all of the geography-related fields at once, so it's find to leave those as is for now.
