README.md cleanup to now refer to existing docs #350

This PR cleans up README.md and moves the smaller bits that didn't have existing documents into their own (eg: packaging/indexing), updates some out of date info / clarifies the shapefile situation a little (points to examples to give users a tiny bit of direction in lieu of us having a spec), etc.

Note: Some of the links refer to documents in other PRs.
