(0) Begroet de cliënt. Greet the cliënt.
(0) Pas handhygiëne toe. Apply handhygiene.
(1) Maak een schoon werkveld en zet daarop de benodigdheden binnen handbereik. Clean your workfield and put your necessities in hands reach.
(2) Trek handschoenen en eventueel schort aan. En zet eventueel een bril op. Put on gloves and possibly an apron. And, if necessary, wear glasses.
(3) Overgiet drie gaasjes met alcohol 70% Cover three gauze pads with 70% alcohol.
(4) Houd de schaar bij een oog vast en desinfecteer de beide bladen met een alcoholgaasje. Hold the scissors by one eye and disinfect both blades with an alcohol swab.
(5) Houd de schaar met het gebruikte gaasje bij een blad vast. Desinfecteer de rest van de schaar met een tweede alcoholgaasje. Hold the scissors with the used gauze by a blade. Disinfect the rest of the scissors with a second alcohol swab
(6) Leg de schaar op het werkveld op een derde alcoholgaasje Place the scissors on the work field on a third alcohol gauze
(7) Overgiet drie gaasjes met alcohol 70% Cover three gauze pads with 70% alcohol.
(8) Houd het pincet bij het handvat vast en desinfecteer beide bladen met een alcoholgaasje. Hold the tweezers by the handle and disinfect both blades with an alcohol swab.
(9) Houd het pincet met het gebruikte gaasje bij een blad vast. Desinfecteer de rest van het pincet met een tweede alcoholgaasje Hold the tweezers with the used gauze by a leaf. Disinfect the rest of the tweezers with a second alcohol swab.
(10) Leg het pincet op het werkveld op een derde alcoholgaasje. Place the tweezers on the work area on a third alcohol gauze.
(11) Vraag de cliënt een comfortabele houding aan te nemen, waarbij de wond goed bereikbaar is. Ask the client to assume a comfortable position with easy access to the wound
(Theory). Zorg dat er geen andere activiteiten in de omgeving plaatsvinden, om stofvorming tegen te gaan. Make sure there are no other activities in the area to prevent dust build-up
(12) Leg een onderlegger rond het werkveld. Place a mat around the work area.
(13) Verwijder het verbandmateriaal en gooi het in de afvalbak. Remove the bandages an throw it in the waste bin.
(14) Inspecteer de wond. Inspect the wound
(Theory) Inspecteer de wond op het volgende: Grootte, diepte, geur, kleur, wondranden en exsudaat (wondvocht).
(15) Trek de handschoenen uit en gooi ze in de afvalbak. Take off gloves and throw them in the waste bin
(16) Pas handhygiene toe. Apply handhygiene
(17) Trek nieuwe handschoenen aan. Put on new gloves
(18) Verwijder het debris, absobeer het exsudaat. Remove the debris, absorb the exudate.
(19) Breng nieuw verbandmateriaal aan volgens afspraak. Apply new dressing material as agreed.
(20) Ruim de materialen op of reinig de materialen volgens voorschrift. Tidy up or clean the materials according to instructions.
(21) Trek de handschoenen uit. Take off the gloves.
(22) Pas handhygiëne toe. Apply handhygiene.
(23) Teken de toedienlijst af of noteer tijdstip, soort medicijn, concentratie, hoeveelheid, wijze van toediening en noteer bevindingen. Sign the administration list or note the time, type of medicine, concentration, amount, method of administration and note down findings.
