Add `study_meta` and `site_meta` files #54

New files should match CTMS export data specs .

Need to get a data pull from CTMS - talk to Andy to figure out where to get it.

These sample files should be good enough to get us started, just need to delete the enrolled_* columns: status_site.csv status_study.csv
