Rename mod to "404 Carpet Addons" #93

Mod is being renamed to 404 carpet addons since most of the development is being done by the 404 members, and not all additions are my own ideas.

Pausing on this since "404" causes issues with identifier naming in Java.

Closing as won't do since it is a lot of effort to rename the mod. Will make it clear that contributions are not just my own, as well as the ideas.
