Clustering added

Description

Renamed the index.js file as app.js and exported a function start from it. Created a new file index.js to support clustering and starting server from each worker.

Fixes #46

Can you explain how is it a level 1 PR?? according to this

it was not a major documention I contributed for a level 3 but at the very least it comes under level 2
