yea i have the same error portal2_d3d9.log here is the log, im using a quest 2 with vd. not sure if that has anything to do with it. Game works fine for a few seconds, skips some frames (i think, hard to describe) and then just crashes.
Same issue, heres my log. portal2_d3d9.log
I was having the same issue but i fixed it (at least for me).
uninstall the mod (remove the 2 files in bin and the two folders in the root) and launch the game with the steam parameters. change the game to windowed mode and not borderless window in settings so you have the windows name bar at the top of the game. close the game, reinstall the mod and launch. not sure if it's just me that this worked but worth a try.

Doesn't seem to do the trick for Samsung Odyessy+ WMR :/
spitting out ideas here, don't know if it would work, but try navigating the menus using only the SteamVR desktop view. might be something with the vr menu that got screwed up in the recent release.

Doesn't matter, it crashes before you're able to attempt to navigate menus.
FOR EVERYONE EXPERIENCING THE ISSUE WHERE IT CRASHES AFTER 10 SECONDS MAKE SURE YOU CLOSE OVERWOLF / ANY OTHER PROGRAM THAT MAY INTERFERE WITH GAMES
Same here, no error just closes on launch valve index.

Does the game open at all. If not:
C:/Program Files (x86)/Steam/
As reported on the Flatscreen to VR Discord channel, multiple users are experiencing game crashes within seconds of launching the game, accompanied by the classic Windows error message 'app is not responding.' The crash seems to occur when the in-game menu is visible, interaction with it is possible for a brief moment before the crash occurs.
