Added apply page

Added an application form, populated with required fields, and added its route
- src/frontend/pages/apply.page.js
- src/frontend/main.js
Added form validation

Description

Application form includes fields detailed in issue
Added form validation to prevent submission of invalid fields (may need to specify required fields)
Added further validation for email and phone numbers using regex
Added dummy data for timezones (ideally this would use data from a database or API)
