sprint 7: fixes for "create request/offer" form

• [x] "active" should be the default
• [x] hide "Revision info" for non-admin users
• [x] change "Worth" to "Price" in the offer form, "Reward" in the request form
• [x] default value of "Worth" should be 1 BZ
• [x] add H1 title on top of the form "Create New [Offer | Request]"
• [x] change order of fields in the form:
  1. title
  2. category
  3. worth
  4. validity
  5. Tell your story
  6. image
  7. status
  8. tags
  9. save
