I have had a few people report this problem of ghost buttons - they are there, but can't be seen. They can be clicked if the person knows what to look for. This happened to me as well on the form development (Chris knows that part because I couldn't find the button to delete a question). Here is an email I received and her picture:
I just wanted to let you know what I see when I login to the site. When I first login and on the welcome page I do not have the 3 lines in the top left corner to select the menu. If I hover over the area is it suppose to be and click then I can still get the menu, just no buttons are displayed. Also once I get into the logic models I don’t have the 4 symbols by the send to admin button. Again if I hover over and click the right spot it will still allow me to go in an edit the report. I just thought that was a odd glitch and wanted to let you know about it. I have signed in and out numerous times on numerous occasions and I still am experiencing the same thing each time.
