URL Adding Form to account Creation Custom #80

When selecting the custom from option it does not show me the active forms.

Took a Deep dive into the code of the Test environment. After many seconds of searching we found that the branchId was set to a constant value of the Dev server. It has been changed to the selected branchId now so it works as intended.
