document the standard base structure for developers #21

The following will be the standard base structure for gloss.theme, this need to be added to the documentation

gloss.xml - gloss rules custom.xml - reserved for customizations custom-css.xml - add custom css here (this is the ONLY rule that can be placed within a containing rule since it should never contain any xsl) grid-webflow.xml - include to support a webflow based grid grid-bootstrap3.xml - include to support a bootstrap3.xml grid grid-??.xml - to support other grids

I'm going to make a judgement call (before we are overrun with opinionated developers :) ) and go with Option A. Next is to document it.
