Fixes #940. Also corrects some mining-speed discrepancies:

Breaking without a tool was slower than vanilla for non-pickaxe blocks.
Breaking with a better tool than necessary wasn't increasing breaking speed.
Breaking speed with a sword doesn't depend in vanilla on the sword's material.
Special handling for the ~~negative~~ extreme hardness of e.g. bedrock.
A rounding error affected some tool/block combinations.
Implements the Efficiency enchantment.
