I think the forms for both gyms and Spa's should be redesigned again to suit each clients specific needs.
The forms must vary with respect to the type of package a user selects. For instance, "Walk-ins" will require the need for a "preferred time/date" option as well as a "payment option" on the form.
But for other membership packages such as a monthly, quarterly or yearly gym membership package, the user is just required to pay a "one time" fee covering the whole package deal and thus allowing him/her to visit the gym any time he/she pleases till the membership expires for that period. Hence there would be no need for a "preferred time/date" option in that scenario.
