Spa/Gym Request Page - Cost Display Issue #164

After a 'package' is chosen on the request page, the cost is not reflected/displayed in the "Cost Box" on the page. It still displays the cost as "0.00"

Why is the cash payment icon not showing? and also the arrow button off and you are not raising this issue?
