Looks like rpc issue. Still happening even after the latest hotfixes

One suggested solution is to first try and fetch from explorer, if fails retry to fetch receipt from blockchain

Won't fix, doesn't happen often and is just a transaction failing, retrying a second time works again (could be as simple as a network issue)
