Interesting, this could definitely be an issue with the randomization system, but it is worth mentioning that there are some “duplicate” status effects and characters (for example I know there are at least 2 different buffs that give invisibility, and void barnacles have 3 different “variants” that are very similar), I’m not aware of any of these “duplicates” for projectiles though.

Didn't know about the void barnacles! Mithrix was his phase 1 version all four times he showed up.
Did some more runs, and it may have something to do with starting them from the end screen instead of going to the main menu first. The RNG may only be getting properly reset on the main menu?
I'm noticing a lot of similarities between runs. With how many different entities, projectiles and status effects there are in the game, I suspect that the method of randomisation is biased or bleeds into other runs to some degree. Examples: 1 run where Engi turrets became Void Barnacles, 1 run where Void Barnacles shot Gummy spawners, and 1 run with both (the latter two slowed the game to a crawl) 2 runs where Engi turrets became Mithrix, only a few runs apart 3 runs where Engi's primary became Void Fiend Flood 3 runs where Shurikens gave invisibility 2 runs where Shurikens gave the Super Massive Leech buff 1 run where Beetle Queen's Beetle Guards became Mithrix, and in the next run a Beetle Guard Horde of Many became Mithrix 1 run where Lemurians shot this projectile, and in the next run Engi's primary was that same projectile
