Survey is putting Country Short Code into org_hq_country field

org_hq_country should have full country name. Updating this involves:

[ ] Modifying location fields in survey to capture full country code
[ ] If short code is being used elsewhere in code for look up values, also add new field to capture short code and update source code references
[ ] Update data currently captured in org_profiles and arcgis_flatfile
