Add document upload instructions text

Add document upload "instructions" to be consistent with previous document up loaders that list the types of documents you can upload there.

-Additional Information: Optional Upload Need to change to "Upload file. Allowed file types .jpg, .pdf, .doc"

-Claim of Acquired Distinctiveness (when yes/whole/evidence are all selected) Need to add "Upload file. Allowed file types .jpg, .pdf, .doc"
