Add support for entering a PIA directly. #280

Allow the user to skip earning record entry and directly input the primary insurance amount. The report is much shorter in this case, because the sections about earnings can't be rendered.

Resolves #65

The entry is shown as an "Alternative Option" on the earnings entry page, suggesting that entering earnings records is recommended.
